Words on board certificate: "Commonwealth of Pennsylvania / Dental Council / To all whom these Presents shall come Greeting: / Whereas, It appears by the report of the Board of Dental Examiners of the State of Pennsylvania that / Alfred Hewish / having given a satisfactory evidence of fitness as to age, character, preliminary education, Dental instruction and all other / matters required by law, was fully examined by the members of the Board of Examiners whose signatures are hereto attached, and / found duly qualified for the practice of Dentistry, he is hereby, in accordance with the provisions of the Act of Assembly approved / July 9, 1877, granted this License to practice Dentistry in the Stae of Pennsylvania. / In Witness Whereof, We, the officers of the said Dental Council, have / hereunto set our hands and the person authorized to affix the Seal / of the Commonwealth has caused the same to be hereto affixed / at Harrisburg, this fifth day of June, 1900. / No. 514 Enrolled in Dental Council Record Book Volume 2 page 14" |
